Please understand that our tests are assembled by hand.  We are not a warehouse.  We use volunteers to keep our costs as low as possible.  Placing your order as far in advance as possible will help us get your order out in a timely fashion.  Thank you.

1.   Complete K2 form

Science with a Mission diagnostics are not for use inside the United States or its territories. So the first thing you must do if you wish to place an order, is to complete the K2 form which states (very repetitively) that you will not use any tests we sell to you inside the US. Put your contact info on the top of page one, then sign and date the bottom (right side) of page two.

Once you have sent in your completed K2 form, feel free to E-mail us your order (all orders must include the info listed in step 3 below).  You only need to fill out the K2 form one time.  You will still be bound by it for any future orders.

In addition to the cost for the diagnostics, you are responsible for the cost of shipping. Most often the tests are shipped to someone in the states who will be traveling to the end-use country.  We can ship by US postal mail or UPS. Please understand that SMI is not responsible for the shipping and cannot provide a shipping cost until after the tests have actually been sent and an exact cost is known. We do not add any handling fees or extra charges. 

Kits do not include lancets. Single-use, spring-loaded safety lancets are available for 20 cents each. The depth of our lancets is 2mm, so if the people you will be testing have thick, leathery skin…our lancets may not be appropriate. 

If the people you serve have thick blood (due to dehydration or other factors) you may wish to purchase additional transfer pipettes. They cost 7 cents each.

PRICING INFORMATION:

HIV diagnostics are sold in kits containing 30 tests, 30 transfer pipettes, and sufficient buffer for 30 tests. The cost is $1 per test, and you must order an increment of 30, so 90 tests would cost $90. You may request the tests to be individually packaged or in 10-packs. The 10-packs are most appropriate for someone ordering a large number (300 or more) of tests. Ten cassettes are contained in a heat-sealed pouch, which also has a Ziploc-style closure. Once you tear open the heat seal, and open the Ziploc, you can remove as many tests as you need. Then re-close the pouch with the Ziploc closure. This method of packaging takes up much less room for traveling and results in far less trash to throw away when all cassettes have been used. However, if you will be using only 1 or 2 tests per day, you should purchase the individually packaged tests.

Typhoid diagnostics are sold in kits containing 10 tests and 10 transfer pipettes. If you will be using stool for the sample, 1mL extraction buffer per test purchased is provided as well. The cost is $1 per test, and the tests must be ordered in increments of 10, so 20  tests would cost $20.

Hepatitis B diagnostics are sold in kits containing 10 tests and 10 transfer pipettes. The cost is $1 per test, and the tests must be ordered in increments of 10, so 30 tests would cost $30.

Hepatitis C diagnostics are sold in kits containing 25 tests, 25 transfer pipettes, and sufficient buffer for 25 tests. The cost is $1 per test, and the tests must be ordered in increments of 25, so 25 tests would cost $25. 

Chlamydia diagnostics are sold in kits containing 25 tests, an extraction buffer, a running buffer, 25 extraction tubes and tips, and 50 female sampling swabs. The cost is $2 per test, and the tests must be ordered in increments of 25, so 25 tests would cost $50. 

Gonorrhea diagnostics are sold in kits containing 25 tests, an extraction buffer, 25 extraction tubes and tips, and 25 female sampling swabs. The cost is $2 per test, and the tests must be ordered in increments of 25, so 50 would cost $100. 

Syphilis diagnostics are sold in kits containing 25 tests, 25 transfer pipettes, and sufficient buffer for 25 tests. The cost is $1 per test, and the tests must be ordered in increments of 25, so 25 tests would cost $25.

3.  Last step to place order

After you have sent the completed K2 form, send an E-mail to alynne@sciencewithamission.org 
to place an order.  Be certain to include the total number for each diagnostic you want (not the number of kits, but the number of tests), your end-use country (for our records), a shipping address, phone number (for use only if there is a problem with the shipment), and the date by which the diagnostics must be received.  Please note that UPS does not deliver on the weekend and tests must be delivered BEFORE the date you are leaving the country.

4.  How to pay

After we ship, we will E-mail the person placing the order with tracking info (if UPS was used) and an invoice.  Payment can be made by check to the address on the form.  (If necessary, you can wire the money, but that will incur a wire transfer fee.)
